Request for Access to GED TEA SE Web Application

IMPORTANT: You must also request access to the GED Application via TEA SE.  See Section 3 Item 1 at the bottom of this form.

To request access to confidential data stored within the GED Database (GEDi), complete this form, print, obtain the required signature of your supervisor, and follow the instructions in Section 3 for submitting the form to the TEA GED Division.

You have requested access to the confidential GED Database.  Specific plans to protect the confidentiality of this information are required in order to process this request.  In lieu of specific plans or documented procedures, please answer the following questions:

 

Who will have direct access to this Confidential information?  If the answer includes personnel not specifically stated on this request form, they must also have a TEASE account with TEA.  If their only access is to be printed reports, then each of them must have a Confidentiality Agreement on file with your organization.

Is the workstation used to access the GED data located in a public or private area?  Could unauthorized personnel see the information displayed on the monitor?

If any of the student-level information is printed (i.e., a report or screen print), where is this printed information kept?  Is it locked in a file cabinet?  If so, who has access to the key?

If other safeguards are used to secure this information, please indicate what these are:

 

I understand that any TEASE username or password assigned to me by TEA is to be considered private and confidential.  I also understand that TEASE username information cannot be released to or shared with another person or entity.

I agree to notify TEA Information Security if my job responsibilities change such that I no longer need access to GEDi or when I terminate employment with my current educational entity for any reason.

I understand that procedures must be in place for monitoring and protecting confidential information.

I understand that any unauthorized disclosure of confidential student information is illegal as provided in the Family Educational Rights and Privacy Act of 1974 (FERPA), and in the implementing of federal regulations found in 34 CFR, Part 99. FERPA is specifically incorporated into the Texas Public Information Act as an exception to records that are subject to disclosures to the public (Government Code, Section 552.026).

In addition, I understand that any datasets or output reports that I, or my authorized representative, may generate using confidential data are to be protected.  I will not distribute to any unauthorized person any datasets or reports I have access to or may generate using confidential data.  I understand that I am responsible for any computer transactions performed as a result of access authorized by use of my TEASE username and password.

I also understand that releasing confidential information is prohibited by the Texas Government code, Section 552.352, and that such an offense constitutes a Class A misdemeanor.

REMINDER

GED data is confidential and must be carefully protected. 

Electronic mail transmittals must be encrypted and paper discards must be shredded.

Section 3: Submitting Your Request  

To complete your application: 

  1. Important: You must have a TEASE account in order to access the GEDi application.

    1. In order to obtain a TEASE account click on the following link: https://seguin.tea.state.tx.us/appsng/um/apply.aspx

    2. If you have a TEASE account, log onto TEASE from https://seguin.tea.state.tx.us/apps/logon.asp   At the upper right hand corner of the screen click on [Add/Modify Application Access] button and select General Education Development.

  2. Print and sign the completed form.  For best printing results, select the Internet Explorer Browser menu option 'File.Page Setup,' remove all variables (e.g., &w, &u) from the Header and Footer, and set all margins to .25 inches. 

  3. Submit the completed and signed form to your supervisor for approval.

  4. Mail a copy or fax all pages of the application to:
