Request for Access to GED TEA SE Web Application

GED Adult Education  or  Probation / Parole
(this form is not for GED Test Centers or ISD/ESCs click here)

To request a new username, or to modify or revoke existing access to GED, complete this form online, obtain the required signatures, and follow instructions completely. To get access to GED system you need to 1. FAX this form and 2. apply on-line via TEASE, see Section 4 for instructions  Your username will expire 12 months from issuance if there is no activity. Please access the GED system at least once every 12 months.

     To request access to GEDi, log onto TEASE from https://seguin.tea.state.tx.us/apps/logon.asp 
     Once you have logged on, at the upper right hand corner of the screen click on [Add/Modify Application Access] button

Section 3:  Request for Access to Confidential Data

You have requested access to confidential information.  Specific plans to protect the confidentiality of this information are needed in order to process this request.  In lieu of specific plans, please provide information requested below:

I agree that my access to GED data (including student name, date of birth, social security number, test score, and any certificate issued) is for the following limited purposes:

 

I understand that any TEASE username or password assigned to me by TEA is to be considered private and confidential.  I also understand that TEASE username information cannot be released to or shared with another person or entity.

I agree that access to confidential data will be limited to the purpose specified above, and I agree to limit the data I view to the data necessary for that purpose.  I understand that procedures must be in place to monitor and protect confidential information.

I agree to notify the Security Administrator at TEA if my job responsibilities change or are terminated, and I no longer need access to this information.  

I understand that any unauthorized disclosure of confidential student information is illegal as provided in the Family Educational Rights and Privacy Act of 1974 (FERPA) and implementing federal regulations found in 34 CFR, Part 99.  FERPA is specially incorporated into the Texas Open Records Act as an exception to records that are subject to disclosures to the public (Government Code, Section 552.026).

In addition, I understand that any data sets or output reports that I, or my authorized representative, may generate using confidential data are to be protected. I will not distribute to any unauthorized person any data sets or reports that I have access to or may generate using confidential data. I understand that I am responsible for any computer transactions performed as a result of access authorized by use of username and password(s).

I also understand that release of confidential student information is prohibited by the Texas Government Code, Section 552.352, and that such an offense constitutes a Class A misdemeanor.

Section 4: Submitting Your Request

To complete your application:


1.  You need a TEASE account in order to access the GEDi application:

    1. In order to obtain a TEASE account click on the following link: https://seguin.tea.state.tx.us/appsng/um/apply.aspx
    2. If you have a TEASE account, log onto TEASE from https://seguin.tea.state.tx.us/apps/logon.asp Once you have logged on, at the upper right hand corner of the screen click on [Add/Modify Application Access] button and select General Education Development.

2.  Print and sign the completed form.

3.  Submit the signed form to your manager or supervisor for approval.

4.  Mail or fax all pages of the application, signed by you and your manager, to the address at the bottom of the page.  If you mail the request, be sure to retain a copy for your records.

If this request for access is approved, you will be notified by email with a username and initial password once you complete the TEASE application.  For speed of processing it is recommended to have completed the TEASE application before submitting this form.
